We offer a GP service to residents of Cheadle Hulme and Handforth. All our services are provided under Stockport NOT Cheshire.
Please download and complete our New Patient Questionnaire (Word document). You do not need proof of ID to register with a GP, but it might help of you have one or more of the following particulary if you would like access to your health information .

When you have completed the forms, you can email it to Hulme Hall Medical Group
Due to high demands on practice workload, once you email the forms into us, it can take up to 21 days to register you on our system. You will receive a welcome email with your named GP, a text message if you are out of the area and login details to access the NHS App. If you do not receive these within 14 days, please contact the practice by telephone.
